Each year, University Interscholastic League (UIL) requires parents of student athletes to provide the school district with proof of a physical exam and additional medical information in order for their children to participate in UIL-sanctioned athletic events. LCISD utilizes the Rank One online platform for this purpose.
This year, in addition to medical history, Rank One has added a "UIL PAPF Eligibility" document to the parent portal. A UIL PAPF form, or Previous Athletic Participation Form, is a document used by the UIL to assess the eligibility of new student athletes for varsity athletic participation in UIL-sanctioned activities. A PAPF is a necessary form for new students or students who have transferred to campuses outside of their attendance zone, but will not be required for the majority of our student athletes. However, because of the district's current attendance zone structure (pre-feeder system), responses to the questions as they are presented by Rank One may result in PAPFs being generated for students who do not need a PAPF (per UIL policy). The district has contacted Rank One regarding this issue; however, adjustments to the online questions are not possible at this time. Instead, we have created instructions along with a flow chart to help parents navigate the questions and determine if a PAPF is truly required for their child.

Log in with previous credentials, or create a new account and add your child.
Once you have logged in, you will be taken to the "Forms Management" landing page. You should see a box containing your child's name, and an "Expand" option in the corner of the box. Click "Expand."
You will now see a list of all electronic documents required for your child. The first document listed is "UIL PAPF Eligibility" - click the blue title of the document.

Once you have completed the "UIL PAPF Eligibility" document, the remainder of the documents are previously-used documents and are self-explanatory. If you require any additional assistance, please do not hesitate to reach out the the LCISD Athletics Department or your child's coach.
