Per TEA Public Health Guidance, Texas school systems must exclude students from attending school in person who have or are suspected of having COVID-19. Exclusion shall continue until the conditions for re-entry are met:
For children with symptoms, at least five days have passed since symptom onset, and fever free, and other symptoms have improved.
Children who test positive for COVID-19 but do not have any symptoms must stay home until at least five days after the day they were tested.
REPORTING A CONFIRMED CASE OF COVID-19
If your child has a confirmed case of COVID-19, please complete the following reporting form (forms are campus-specific):
Lubbock-Cooper High School
New Hope Academy
Lubbock-Cooper Middle School
Laura Bush Middle School
Central Elementary
East Elementary
North Elementary
South Elementary
West Elementary
Please note, the form will require evidence of a positive case. Evidence may be one of the following:
A signed and dated doctor's note which confirms a case of COVID-19. Note must also include student's name.
A confirmation document from a testing site. Document must include student's name and the date of positive.
A photograph of an at-home test which reads positive. Student's name and the date of positive must be written on the test in permanent marker.
Absences resulting from a confirmed case of COVID-19 will be unexcused until a COVID-19 Reporting Form is completed for the student. In addition to completing the COVID-19 Confirmed Case Form, you must also notify your campus of the absence. The forms above do not take the place of notification of absence.
